Certification

I have read and agree to observe and abide by the requirements set out in the above documents, particularly in respect of Activities M and M1, relating to the protection of children from harm: to refuse sales of alcohol to underage persons, persons considered drunk or persons that are purchasing alcohol for another person considered drunk, and to ensure that there is no drunken or disorderly behaviour.  No alcohol shall be consumed by those aged under 18, unless they are eating a table meal, in which case no alcohol shall be consumed by those under 16.

I will assume responsibility for so doing throughout the event(s).  In the case of Activities M and M1 the following persons will be responsible for supervising the bar.  They must at all times ensure that the provisions of the Operating Schedule are adhered to.   No other person will be permitted to sell alcohol.  The authority for these persons to supervise the bar must be given by a Personal Licence Holder in writing before the event.
